How to Get Involved
Whether you are interested in participating in one of our research projects or in being seen by one of our private doctors, we ask that you fill out a questionnaire to provide us with information about your health status. This information is kept strictly confidential.
If you are interested in participating in research, you should either be a patient with severe pain or fatigue that has no apparent medical cause or you should be a healthy person interested in advancing science to better understand the causes of these ailments. If you are a patient and know of a friend or relative - in good health - who would also like to participate in our studies, we would like to hear from you and from him or her also. There are no costs to participating in research, and monetary compensation for your time is often available.
If you are interested in research but also wish to be seen by one of the private care doctors, we urge that you do the research piece first because many of the studies cannot be done if you are taking medicine. Unfortunately, we cannot study every person who wishes to be a research participant due to the specifics of our research protocols. We study about one in seven people who complete our questionnaire.
If you are not interested in participating in research but wish to see one of the physicians about your illness, we still need for you to fill out our health information packet.
For your convenience, there are three ways for you to obtain our research questionnaire.
- Download the Screening Form electronically. This is the quickest way to get information about your symptoms to our Center.
Once you fill out the forms, please fax them to 973-972-7656.
You may also mail the forms to:
Attn: Jennifer FitzGibbons, APN
UMDNJ
Pain & Fatigue Study Center
30 Bergen Street, ADMC/1618
Newark, NJ 07101
- Call (973) 972-4800 and leave your name and address and we will mail you a questionnaire. Please indicate whether you want the form for a person with CFS or FM, or for a Healthy Volunteer.
